To make a referral to Community and Nursing Services Division (CNSD) for Pregnancy and Parenting Support:

• Online secure CNSD Referral Portal; submit any attachments via fax to 714-834-7780 or secure email to PublicHealthNursing@ochca.com.
• CNSD Referral Form (fillable pdf for fax or secure email) or call (714) 834-7747 for more information.

Public health staff provide free personalized support and can help with the following:

• Pregnancy and health concerns
• Health education
• Monitoring your baby’s growth and development
• Connecting you with services and resources
• Support with health, emotional, alcohol or substance use concerns

Programs include:

• Adolescent Family Life Program (AFLP)
• Nurse-Family Partnership® (NFP)
• Perinatal Assessment and Coordination Team (PACT)
